The thing that imo makes the lady in waiting "deserve" the slap is not only the powder but also the constant delaying of lihuas treatment due to her arrogance. There's also her kicking maomao out every chance she gets, as well as disobeying the emperors orders. Plus given that Maomao was physically kicked out multiple times, that implies that they put hands on her. The slap was 100% deserved.

The emperor or nobles of high rank can kill low class commoners for as small reason as dropping a vase. Normally in the case of doctors assigned to high ranking people, if the patient die they will likely be executed to die along with them. And if low class commoners make the emperor angry, all their family and relatives can be executed (the emperor have the right to, but not all of them will do this).
